French pirate Emanuel Wynne was supposedly the first pirate to fly a skull and crossbones flag in 1700. Wynne raised his flag off the northern coast of Africa-- his flag design also included an hourglass which implied that time was running out. For many years after, pirate captains customized their own personal flags, adding swords, spears and bleeding hearts.
